# Create form

> Creates a Tally form from typed blocks or a template.

`tally.createForm` creates a Tally form from typed blocks or a template.

## Example

```ts automations/tally-create-form.automation.ts theme={null}
import { automation, onHttpRequest } from "automate.ax"
import { tally } from "automate.ax/tally"

export default automation("Create form", () => {
  onHttpRequest({ scope: "automation" })

  tally.createForm({
    status: "DRAFT",
    blocks: [],
    workspaceId: "workspace-id",
  })
})
```

## Inputs

`status` and the ordered `blocks` array are required. Optionally pass `workspaceId`, `folderId`, `templateId`, and form `settings`. Block objects use Tally's discriminated block types.

Pass `account` to select a named Tally connection.

## Output

Returns the created form.

Creating a form is not replay-safe, so the action is not retried after an uncertain provider response.
